2024-2025 Membership Registration Form

Please complete all sections (one per family) and pay the dues including a $50 non-refundable deposit per student prior to the first rehearsal.

Does your student(s) have any allergies, behavioral challenges, or medical issues our staff need to be aware of? We want to be as inclusive as possible and ensure success for each individual as well as our entire ensemble. Please share anything that may be relevant below, or contact our Business Manager directly.

I agree to pay my membership dues in full or sign up for monthly payments before the first rehearsal.

I agree to attend all rehearsals and performances in accordance with the schedule and communicating any known absences. Learning music and choreography missed due to absence will be my responsibility prior to the next rehearsal. I will be on time for arrival to and departure from all rehearsals, performances and special events. If I am going to leave early or arrive late, I will notify a staff member first. I understand that after 3 absences my role may be given to someone else.

I agree to whenever possible, participate in the responsibilities and duties of, at least, two support/volunteer committees for the activities of ECC.Parent Support Roles

I agree to support fundraising events for the benefit of ECC.

Do you have any affiliation with a business that might be interested in being a corporate sponsor, advertising in an ECC program, or donating to our annual Silent Auction?

I agree to be responsible for the uniforms or costumes necessary to the production of the program and to pay a replacement fee for any uniform item not returned at the end of the season.

I agree to conduct myself at all times in a manner consistent with the honor and privilege associated with membership in this organization and abide by the rules and expectations set forth by its governing Board and Directors in the Membership Handbook.View Membership Handbook.
